CLINICAL DEPARTMENT

**All clinical field positions require access to a cell phone, high speed internet and experience with computer technology. Field positions are available in Baltimore City, Baltimore County, and the surrounding areas.

Field Registered Nurse (F/T and per diem) **

Qualifications:

  • Home care experience required
  • Knowledge of Medicare and OASIS documentation preferred

Social Worker-Community Health
Qualifications:

  • Education - Master of Social Work (MSW), LCSW in the State of Maryland -Experience working with the elderly and chronically ill in a long-term setting. Community health experience preferred
  • Serves as liaison between the Agency and the medical community in order to promote continuity of care
  • Provides social work services in the development of the plan of treatment for patients
  • Meets the concrete needs of the patient as they arise in the home (e.g., finances, medical equipment, transportation) by utilizing appropriate community services
  • Evaluates and intervenes in the patient’s social situation by working with the family, providing emotional support, counseling regarding the impact of illness and helping with behavior changes as requested by patient, family or physician
  • Makes referrals to appropriate community resources for services and provides follow-up to insure their provision
  • Prepares clinical and progress notes that become part of the patient record

ADMINISTRATIVE DEPARTMENT

Q/A RN (Home Health Care)

  • Demonstrates a working knowledge of Medicare guidelines (recent) for home health coverage and compliance.
  • Oasis documentation training
  • Experience with OASIS scrubbing software
  • Review each patient chart to ensure all documentation has been obtained to support claim for payment.
  • Informs the Clinical Managers of problem areas regarding documentation and standards of care, as indicated in patients' charts
  • Assists with Medicare requests for additional information, filing Reconsiderations, Reopening of denials as directed.

PRIVATE DUTY DEPARTMENT
Home Health Aide (perdiem)
Qualifications:

  • Current Maryland CNA/GNA and CPR certification required
  • Previous experience with private duty preferred
